Schedule/Hours:

Key Qualifications:
- Active, unrestricted LPN license
- At least one year of recent, hands-on experience in a home health setting
- Thorough understanding and proficiency in HomeCare HomeBase (HCHB) EMR
- Strong clinical assessment, documentation, and communication abilities
- Effective organization and the ability to manage a patient caseload independently
- Reliable transportation is essential for traveling to patient homes
Core Responsibilities:
- Deliver skilled nursing care based on individualized care plans in patients homes
- Monitor and document patient condition; update care plans as patient needs evolve
- Administer medications and carry out physician-ordered treatments within LPN scope
- Educate patients and families on managing health conditions and preventative practices
- Maintain thorough, timely medical records using the HCHB platform
- Collaborate closely with registered nurses, therapists, and interdisciplinary team members to ensure coordinated, high-quality care
