Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional flexibility required based on patient needs
Key Responsibilities:
- Conduct skilled nursing visits, developing and implementing individualized care plans for home health patients
- Administer medications, perform wound care, manage IV therapy, and provide other nursing interventions as prescribed
- Monitor changes in patient conditions and promptly communicate findings to the RN Case Manager or physician
- Educate patients and caregivers on disease management, medication administration, and safe home care practices
- Comprehensively document assessments, interventions, and communications using Home Care Home Base (HCHB)
- Collaborate closely with the interdisciplinary healthcare team to coordinate and deliver effective patient care
- Uphold infection control protocols and ensure a safe home environment for all patients
- Attend case conferences, participate in quality improvement initiatives, and complete mandatory training
Qualifications:
- Active, unrestricted LPN license in the state of practice
- At least one year of recent home health, hospice, or community-based nursing experience
- Proficiency with the Home Care Home Base (HCHB) electronic health record system
- Valid drivers license and reliable, insured transportation
- Current Basic Life Support (BLS) certification
- Strong assessment, documentation, and communication abilities
- Capacity to work independently in a field setting and effectively manage multiple patients
- Preferred: Prior experience in Medicare-certified home health agencies and advanced compliance knowledge
