Schedule/Hours:

Qualifications:

  • Active LPN license required
  • At least one year of recent experience in home health care
  • Proficiency with HomeCare HomeBase (HCHB) electronic medical record system
  • Strong clinical assessment, documentation, and communication skills
  • Reliable transportation for travel to patient homes
  • Ability to independently manage a caseload and adapt to changing situations

Key Responsibilities:

  • Deliver skilled nursing care to patients at home following individualized care plans
  • Monitor patient status, perform clinical assessments, and update care documentation in HCHB
  • Administer prescribed medications and treatments in accordance with state rules
  • Provide patient and family education on disease prevention, medication management, and self-care techniques
  • Maintain thorough, accurate medical records within the HCHB EMR platform
  • Coordinate and communicate with RNs, therapists, and other healthcare staff to ensure holistic patient care
