  • Administer medications and treatments in line with state guidelines
  • Educate patients and families on ongoing care management and prevention
  • Accurately maintain records through HomeCare HomeBase (HCHB) EMR
  • Coordinate care with registered nurses, therapists, and healthcare team members to ensure comprehensive support

Qualifications:

  • Active LPN license
  • At least one year of recent home health experience
  • Proficiency in HomeCare HomeBase (HCHB) electronic medical records
  • Strong clinical assessment skills and detail-oriented documentation
  • Reliable transportation for travel between patient homes
  • Demonstrated ability to work independently and manage a caseload efficiently
