Job Description

Deliver compassionate and expert care as a Licensed Practical Nurse specializing in home health. This contract position is focused on supporting patients health and independence in their own homes, while engaging with a dynamic and collaborative healthcare team. As a field-based nurse, you will be responsible for a manageable caseload and play a crucial role in individualized patient care and recovery.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM
• Occasional flexibility to meet patient needs
• Field-based in patients homes, manageable daily visits (approximately 35 per day)

Key Responsibilities:

• Perform skilled nursing visits to develop, implement, and update individualized care plans
• Administer prescribed medications, provide wound care, and manage prescribed therapies
• Monitor patient conditions and communicate changes to the supervising RN or physician
• Deliver education to patients and caregivers on managing conditions, medications, and self-care
• Accurately document all assessments and interventions using Home Care Home Base (HCHB)
• Collaborate with the interdisciplinary team to ensure comprehensive, compliant patient care
• Maintain infection control and safety protocols at all times
• Participate in case conferences, required trainings, and quality improvement initiatives

Qualifications:

• Current, active LPN license in the state
• At least one year of home health, hospice, or community-based care experience
• Proficiency with Home Care Home Base (HCHB)
• Valid drivers license, reliable transportation, and insurance
• Current BLS certification
• Effective time management, strong clinical skills, and the ability to work independently

Preferred Experience:

• Medicare-certified home health agency background
• In-depth knowledge of home health compliance standards
• Excellent communication and patient-focused approach

$1,309 - $1,384/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
