Deliver compassionate, high-quality nursing care as a Home Health Licensed Practical Nurse (LPN) providing patient-centered services in their homes. This contract position offers the opportunity to manage a dynamic caseload within an assigned territory, promoting patient independence, education, and recovery. Emphasis is placed on strong communication with patients, families, and the interdisciplinary healthcare team to ensure coordinated, compliant care.
Location:
You will serve patients in and around Carlisle, KY, a charming community known for its warm hospitality and serene rural landscapes. This setting provides an inviting environment for home-based care, allowing for meaningful patient interactions within peaceful residential neighborhoods.
Schedule & Workload:
- Full-time contract role
- Monday to Friday, 8:00 AM 5:00 PM (with occasional flexibility)
- Approximately 35 patient visits per day, totaling 2532 points weekly
Key Responsibilities:
- Conduct skilled nursing visits to assess, plan, and implement individualized home health care
- Administer medications, perform wound care, manage IV therapy, and other clinical interventions
- Monitor patient conditions, reporting changes to RN Case Manager or physicians promptly
- Educate patients and caregivers on health management and self-care techniques
- Maintain accurate documentation using Home Care Home Base (HCHB)
- Collaborate with the healthcare team to coordinate care and uphold compliance standards
- Follow infection control protocols and ensure patient safety
- Participate in case conferences, quality improvement, and ongoing training
Qualifications & Experience:
- Active, unrestricted LPN license in the state of practice
- Minimum 1 year experience in home health, hospice, or community-based care
- Proficient with Home Care Home Base (HCHB) charting
- Valid driver's license, reliable vehicle, and current insurance
- Current BLS certification
- Strong clinical judgment, time management, and communication skills
- Ability to work independently in a field environment
Preferred:
- Experience with Medicare-certified home health agencies
- Knowledge of home health compliance and documentation standards
- Patient-centered approach and excellent interpersonal skills
