Job Description
Take on a rewarding role delivering compassionate care as a Home Health Licensed Practical Nurse (LPN), providing direct nursing services to homebound individuals. This full-time contract position involves performing skilled nursing visits within a designated territory, using your expertise to help patients regain independence and optimize their health outcomes.
Schedule:
- Monday to Friday, 8:00 AM 5:00 PM
- Occasional flexibility required to accommodate patient needs
Key Qualifications:
- Active, unrestricted LPN license in the state of practice
- Minimum one year of experience in home health, hospice, or community-based care
- Proficiency with Home Care Home Base (HCHB) charting system
- Current BLS certification
- Valid drivers license, reliable vehicle, and up-to-date auto insurance
- Excellent time management, clinical judgment, and interpersonal skills
- Ability to work independently and manage a 2532 point weekly caseload (approx. 35 visits/day)
Preferred Experience:
- Previous work with Medicare-certified home health agencies
- Strong understanding of compliance and home health documentation standards
- Patient-centric communication and a collaborative mindset
Primary Responsibilities:
- Deliver comprehensive nursing assessments, develop and implement care plans, and administer medications as ordered
- Provide wound care, IV therapy, and other clinical interventions in accordance with care plans
- Monitor patient status and promptly relay any changes to the RN Case Manager or physician
- Educate patients and caregivers on disease management, medication protocols, and safe self-care
- Accurately document all visits and care in Home Care Home Base (HCHB)
- Strictly follow infection control protocols and support high standards of patient safety
- Participate in team meetings, trainings, and quality improvement initiatives
