Job Description

An opportunity is available for an experienced and compassionate Licensed Practical Nurse (LPN) specializing in home health care. In this contract position, you will deliver patient-centered nursing care to individuals in their homes, playing a vital role in their recovery, education, and independence. This role also emphasizes strong communication with patients, caregivers, and the wider healthcare team.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM (occasional flexibility for patient needs)

Key Responsibilities:

  • Conduct skilled nursing visits and develop individualized care plans in patients homes
  • Administer medications, perform wound care, and manage IV therapy and other physician-ordered treatments
  • Monitor patient progress, reporting changes to the RN Case Manager or physician
  • Educate patients and caregivers on disease processes, medication administration, and self-care techniques
  • Complete accurate, timely documentation of care and communication in Home Care Home Base (HCHB)
  • Coordinate with the interdisciplinary team to ensure compliance with the plan of care
  • Uphold infection control standards and maintain patient safety at all times
  • Participate in case conferences, quality improvement initiatives, and training as required

Qualifications:

  • Active, unrestricted LPN license in the state of practice
  • At least one year of experience in home health, hospice, or similar community-based setting
  • Proficiency in Home Care Home Base (HCHB) required
  • Valid drivers license, reliable vehicle, and current insurance
  • Current BLS certification
  • Excellent clinical judgment, time management, and independent decision-making skills

Preferred:

  • Previous work with Medicare-certified home health agencies
  • In-depth knowledge of home health compliance and documentation requirements
  • Superior interpersonal and patient-centered care skills
