An opportunity is available for an experienced and compassionate Licensed Practical Nurse (LPN) specializing in home health care. In this contract position, you will deliver patient-centered nursing care to individuals in their homes, playing a vital role in their recovery, education, and independence. This role also emphasizes strong communication with patients, caregivers, and the wider healthcare team.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM (occasional flexibility for patient needs)
Key Responsibilities:
- Conduct skilled nursing visits and develop individualized care plans in patients homes
- Administer medications, perform wound care, and manage IV therapy and other physician-ordered treatments
- Monitor patient progress, reporting changes to the RN Case Manager or physician
- Educate patients and caregivers on disease processes, medication administration, and self-care techniques
- Complete accurate, timely documentation of care and communication in Home Care Home Base (HCHB)
- Coordinate with the interdisciplinary team to ensure compliance with the plan of care
- Uphold infection control standards and maintain patient safety at all times
- Participate in case conferences, quality improvement initiatives, and training as required
Qualifications:
- Active, unrestricted LPN license in the state of practice
- At least one year of experience in home health, hospice, or similar community-based setting
- Proficiency in Home Care Home Base (HCHB) required
- Valid drivers license, reliable vehicle, and current insurance
- Current BLS certification
- Excellent clinical judgment, time management, and independent decision-making skills
Preferred:
- Previous work with Medicare-certified home health agencies
- In-depth knowledge of home health compliance and documentation requirements
- Superior interpersonal and patient-centered care skills
