Job Description

Make a meaningful impact by providing essential nursing care to patients in a home health setting. As a Licensed Practical Nurse, youll utilize your clinical expertise and compassion to support individuals on their path to independence and recovery. This full-time contract position is field-based, serving patients within an assigned territory Monday through Friday, 8:00 AM 5:00 PM, with occasional flexibility to accommodate patient needs. Youll manage a caseload of 2532 points per week (about 35 patient visits per day), leveraging your experience with Home Care Home Base (HCHB) to deliver and document high-quality care.

Qualifications & Experience:

  • Active, unrestricted LPN license in the state of practice
  • Minimum 1 year of home health, hospice, or similar community-based nursing experience
  • Proficiency with Home Care Home Base (HCHB) charting
  • Current BLS (Basic Life Support) certification
  • Valid drivers license, reliable vehicle, and current auto insurance
  • Exceptional clinical judgment, time management, and communication skills
  • Ability to work independently with a dynamic caseload
  • Preferred: Experience with Medicare-certified home health agencies and a strong understanding of home health compliance/documentation standards

Key Responsibilities:

  • Provide skilled nursing visits and implement individualized care plans
  • Administer medications, wound care, IV therapy, and other clinical interventions as ordered
  • Report condition changes promptly to the RN Case Manager or physician
  • Educate patients and caregivers on disease management and self-care
  • Document assessments and interventions accurately using HCHB
  • Collaborate with the interdisciplinary team to coordinate patient care
  • Maintain infection control and ensure patient safety at all times
  • Participate in case conferences, quality improvement activities, and ongoing training
