Job Description

Deliver vital patient care in the home setting as an experienced Licensed Practical Nurse. In this contract role, you will focus on providing individualized nursing services, promoting patient autonomy, and educating patients and familiesall while coordinating with a multidisciplinary healthcare team. This position is ideal for LPNs skilled in Home Care Home Base (HCHB) and experienced in navigating the unique demands of home health settings.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM (occasional flexibility based on patient needs)

Role Responsibilities:

  • Manage a caseload of 2532 points per week, performing approximately 35 patient visits per day in patients homes.
  • Conduct skilled nursing assessments, develop and implement care plans, and respond to changes in patient conditions.
  • Administer medications, manage wound and IV care, and execute other clinical interventions as prescribed.
  • Document all care, assessments, and communications accurately using HCHB.
  • Educate patients and their families about disease processes, medication adherence, and self-care techniques.
  • Maintain constant communication with the interdisciplinary healthcare team and participate in case conferences.
  • Strictly adhere to infection control and patient safety protocols at every visit.
  • Attend relevant training and participate in quality improvement initiatives.

Minimum Qualifications:

  • Active, unrestricted LPN license for the state of practice.
  • At least 1 year of experience in home health, hospice, or similar community-based care.
  • Demonstrated proficiency in Home Care Home Base (HCHB).
  • Current BLS certification.
  • Valid drivers license, reliable transportation, and current vehicle insurance.
  • Excellent clinical judgment, organizational, and communication skills.
  • Ability to work independently and efficiently in a field-based environment.

Preferred:

  • Background with Medicare-certified home health agencies.
  • Familiarity with compliance and documentation specific to home health.
  • Strong patient-centered approach and interpersonal abilities.
