Delivering vital care to individuals in need, this field-based role offers experienced LPNs the opportunity to provide comprehensive home health services. The position centers on patient education, independence, and recoveryall supported by timely and thorough documentation using Home Care Home Base (HCHB).
Schedule:
- Monday Friday, 8:00 AM 5:00 PM (occasional flexibility to accommodate patient needs)
Key Responsibilities:
- Complete skilled nursing assessments and develop personalized care plans for home health patients
- Administer medications, perform wound care, and manage IV therapy per physician orders
- Closely monitor patient conditions, reporting changes promptly to the RN Case Manager or physician
- Deliver education to patients and caregivers on disease management, medication, and self-care
- Accurately document all care activities using Home Care Home Base (HCHB)
- Collaborate effectively with the interdisciplinary care team to ensure high-quality, compliant care
- Uphold infection control and patient safety standards
- Actively participate in case conferences, training, and quality improvement initiatives
Qualifications:
- Current, unrestricted LPN license in the state of practice
- At least 1 year of home health, hospice, or similar community-based experience
- Proficiency with Home Care Home Base (HCHB) charting
- Valid drivers license and reliable transportation with insurance
- Active BLS (Basic Life Support) certification
- Excellent clinical, organizational, and communication skills
- Ability to independently manage a dynamic field caseload
Preferred Experience:
- Familiarity with Medicare-certified home health agencies
- Strong understanding of compliance and documentation standards
- Demonstrated patient-centered care and interpersonal skills
