An exciting opportunity is available for a skilled and compassionate Licensed Practical Nurse (LPN) to provide quality care to patients in the comfort of their own homes. This contract role offers the chance to make a meaningful impact, fostering patient independence and recovery while maintaining strong communication with both families and the interdisciplinary healthcare team.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Occasional flexibility may be needed to accommodate patient needs

Key Qualifications:

• Active, unrestricted LPN license in West Virginia
• At least 1 year of experience in home health, hospice, or a similar community-based care setting
• Proficient with Home Care Home Base (HCHB) software
• Valid drivers license, reliable transportation, and current auto insurance
• Current Basic Life Support (BLS) certification
• Excellent clinical judgment, organization, and communication skills
• Ability to manage a caseload independently in a field-based environment

Preferred Experience:

• Previous work in Medicare-certified home health settings
• Thorough understanding of home health compliance and documentation
• Strong interpersonal skills and a patient-focused attitude

Responsibilities:

• Conduct skilled nursing visits, assessing and executing individualized care plans
• Administer medications, manage IV therapy, and perform wound care according to physician orders
• Monitor and document changes in patient condition, reporting to RN Case Manager or physician as needed
• Educate patients and caregivers on medication management, self-care, and disease processes
• Accurately document all assessments and interventions in HCHB system
• Collaborate with care team and participate in conferences and quality improvement initiatives
• Uphold infection control and patient safety standards at all times
