A passionate Licensed Practical Nurse (LPN) is needed for a contract role supporting patients in their homes. Qualifications:
- Active and unrestricted LPN license
- At least one year of recent experience in home health
- Proficient using HomeCare HomeBase (HCHB) EMR platform
- Excellent clinical assessment and documentation abilities
- Demonstrated reliability and strong organizational skills for managing home visits
- Must have reliable transportation for patient care visits
- Capability to work autonomously while collaborating with multidisciplinary healthcare teams
Responsibilities:
- Deliver skilled nursing care in patient homes, following individualized care plans
- Accurately monitor patient progress and revise care plans as appropriate
- Administer medications and perform treatments within state LPN guidelines
- Educate patients and their families on disease management and preventive care
- Document all care provided using HomeCare HomeBase (HCHB) EMR system
- Collaborate and coordinate with RNs, therapists, and other healthcare professionals to ensure comprehensive patient care
