An exciting opportunity is available for a skilled Licensed Practical Nurse (LPN) with experience in home health to deliver essential care to patients in a field-based setting. This contract position focuses on providing personalized, high-quality nursing care while supporting patient recovery, independence, and education.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Flexibility for occasional patient needs
• 2532 points per week (approximately 35 patient visits per day)

Responsibilities:

• Perform comprehensive nursing visits, assess patient needs, and create individualized care plans
• Deliver medication administration, wound care, IV therapy, and other skilled clinical interventions as ordered
• Monitor patient conditions; promptly communicate changes to RN Case Managers or physicians
• Educate patients and caregivers on health management and disease processes
• Ensure meticulous documentation and compliance using Home Care Home Base (HCHB)
• Collaborate with the interdisciplinary team to coordinate effective patient care
• Maintain infection prevention protocols and prioritize patient safety
• Engage in case conferences, training, and ongoing quality improvement initiatives

Requirements:

• Active, unrestricted LPN license in the state of practice
• At least one year of recent home health, hospice, or related community-based clinical experience
• Proficiency with Home Care Home Base (HCHB)
• Current BLS certification
• Valid drivers license, reliable transportation, and current auto insurance
• Strong clinical, time management, and communication skills
• Ability to thrive independently in a dynamic, field-based role

Preferred:

• Experience working with Medicare-certified home health agencies
• Advanced understanding of compliance and documentation protocols in home health
• Compassionate, patient-first care philosophy and excellent interpersonal skills
