Job Description
Deliver compassionate, high-quality nursing care as a Home Health Licensed Practical Nurse (LPN) providing support to patients in their own homes. In this full-time contract role, youll work closely with patients, caregivers, and interdisciplinary healthcare teams to foster independence, promote recovery, and ensure the best possible outcomes for every patient in your care.
Schedule/Hours:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional flexibility to accommodate patient needs
Key Responsibilities:
- Complete skilled nursing visits, assessing and implementing individualized care plans for home health patients
- Administer medications, perform wound care, manage IV therapy, and deliver other nursing interventions as prescribed
- Monitor changes in health status and report findings promptly to the RN Case Manager or physician
- Provide education to patients and caregivers on disease management, medications, and home safety
- Accurately document all care activities using Home Care Home Base (HCHB) electronic charting system
- Collaborate with an interdisciplinary team to coordinate patient care and ensure compliance with care plans
- Uphold infection control practices and maintain patient safety at all times
- Participate in case conferences, quality improvement initiatives, and ongoing staff training
Required Qualifications:
- Active and unrestricted LPN license in state of practice
- At least 1 year of recent home health, hospice, or community-based nursing experience
- Proficiency in Home Care Home Base (HCHB)
- Valid drivers license, reliable insured vehicle
- Current BLS certification
- Solid clinical judgment, organizational, and communication skills
- Ability to work independently in a field-based environment
Preferred Qualifications:
- Experience with Medicare-certified home health agencies
- Strong understanding of compliance and documentation standards
