Provide compassionate, patient-centered nursing care as a Home Health Licensed Practical Nurse (LPN) serving individuals in their homes. This role involves managing a caseload of patients, delivering skilled nursing interventions, and ensuring accurate documentation in compliance with home health standards. The ideal candidate excels in patient education, clinical assessment, and communication with the healthcare team to promote independence and recovery.

Responsibilities:

• Perform skilled nursing visits, including medication administration, wound care, and IV therapy management.
• Assess and monitor patient conditions, communicating changes promptly to the RN Case Manager or physician.
• Educate patients and caregivers on disease management, medication use, and self-care techniques.
• Maintain accurate, timely documentation using Home Care Home Base (HCHB).
• Collaborate with interdisciplinary teams to coordinate and update care plans.
• Uphold infection control and patient safety standards.
• Participate in case conferences, quality improvement initiatives, and ongoing training.

Qualifications:

• Active, unrestricted LPN license in the practicing state.
• Minimum of 1 year of experience in home health, hospice, or community-based care.
• Proficiency with Home Care Home Base (HCHB) documentation system.
• Current BLS certification.
• Valid drivers license with reliable insured transportation.
• Strong clinical judgment, excellent time management, and effective communication skills.
• Ability to work independently managing a dynamic caseload in a field-based setting.

Preferred:

• Experience with Medicare-certified home health agencies.
• In-depth knowledge of home health compliance and documentation standards.
• Patient-centered approach with strong interpersonal skills.

Job Details:

• Contract type: Full-time, contract.
• Schedule: Monday Friday, 8:00 AM 5:00 PM (with occasional flexibility).
• Patient visits: Approximately 35 per day (2532 points weekly).
• Work setting: Patient homes within assigned territory.
• Reliable transportation required; mileage reimbursement typically available.
