Deliver meaningful care where it matters mostat home. Skilled and dedicated Licensed Practical Nurses (LPNs) are needed to provide expert support and clinical care to patients in a home health setting. Step into a pivotal role promoting recovery, education, and independence, while closely communicating with patients, families, and a multidisciplinary healthcare team.
Schedule:
- Monday to Friday, 8:00 AM 5:00 PM
- Flexibility required for occasional patient needs
Key Qualifications:
- Active, unrestricted LPN license for the state
- At least 1 year of prior home health, hospice, or related community care experience
- Strong proficiency and usage history with Home Care Home Base (HCHB)
- Valid drivers license, reliable transportation, and current auto insurance
- BLS (Basic Life Support) certification
- Excellent clinical assessment, time management, and communication abilities
- Capability to work independently in patient homes, managing a caseload of 2532 points/week
Preferred Experience:
- Work in Medicare-certified home health agencies
- Deep knowledge of compliance, charting, and documentation for home health
- Effective patient and caregiver teaching skills and teamwork mindset
Responsibilities:
- Deliver skilled nursing visits for assessment and implementation of individualized home care plans
- Administer prescribed medications, provide wound care, IV therapy, and clinical interventions
- Closely observe patients and report changes to the RN Case Manager or physician
- Educate patients and caregivers on disease management and self-care
- Complete accurate electronic charting in HCHB, ensuring documentation integrity and compliance
- Actively participate in care conferences, training, and quality improvement initiatives
