Job Description

An experienced Licensed Practical Nurse (LPN) with a passion for delivering compassionate patient care in home settings is essential for this opportunity. This role requires providing skilled nursing services to patients within their residences, supporting their health, independence, and recovery through close communication with patients, families, and healthcare professionals.

Work Schedule and Type:

  • Full-time contract position
  • Monday through Friday, 8:00 AM to 5:00 PM, with occasional flexibility based on patient needs
  • Approximately 3 to 5 patient visits per day, managing a caseload totaling 2532 points weekly
  • Field-based care within assigned territory
  • Utilization of Home Care Home Base (HCHB) system for documentation
  • Reliable transportation is necessary; mileage reimbursement commonly provided

Key Responsibilities:

  • Perform skilled nursing assessments, develop and execute individualized care plans
  • Administer medications, wound care, IV therapy, and other clinical interventions as prescribed
  • Monitor patient conditions and communicate changes promptly to supervising RN or physician
  • Educate patients and caregivers on disease management, medication adherence, and self-care skills
  • Accurately document all care activities and maintain compliance with regulatory standards using HCHB
  • Collaborate effectively with interdisciplinary teams to ensure coordinated patient care
  • Uphold infection control practices and ensure patient safety
  • Participate in case conferences, quality initiatives, and ongoing training

Qualifications:

  • Active, unrestricted LPN license in the practicing state
  • Minimum one year of experience in home health, hospice, or community care
  • Proficiency with Home Care Home Base (HCHB) software
  • Valid drivers license, reliable insured vehicle
  • Current Basic Life Support (BLS) certification
  • Strong clinical decision-making, communication, and time management skills
  • Ability to work independently managing a dynamic caseload in a field setting

Preferred Qualifications:

  • Experience with Medicare-certified home health agencies
  • Understanding of home health compliance and documentation standards
  • Excellent interpersonal abilities and a patient-focused approach
