An experienced Licensed Practical Nurse (LPN) with a passion for home health nursing is needed to deliver compassionate, patient-centered care within a community setting. This contract role focuses on fostering patient independence and recovery through skilled clinical interventions while maintaining clear communication with patients, their families, and the healthcare team.

Responsibilities:

• Conduct skilled nursing visits to assess, develop, and implement individualized care plans
• Administer medications, perform wound care, and manage IV therapy per orders
• Monitor patient condition changes and report promptly to RN Case Manager or physician
• Educate patients and caregivers on disease processes, medication management, and self-care techniques
• Document all care and communication accurately using Home Care Home Base (HCHB)
• Collaborate with interdisciplinary team members to coordinate care and ensure plan compliance
• Uphold infection control standards and maintain patient safety throughout visits
• Participate in case conferences, quality improvement efforts, and mandatory training

Qualifications:

• Active and unrestricted LPN license in the state of practice
• Minimum one years experience in home health, hospice, or community-based care
• Proficiency with Home Care Home Base (HCHB) charting system
• Valid driver's license, insured reliable vehicle, and ability to travel within assigned territory
• Current Basic Life Support (BLS) certification
• Strong clinical judgment, communication, and time management skills
• Ability to work independently managing a dynamic caseload

Preferred Qualifications:

• Experience with Medicare-certified home health agencies
• Comprehensive knowledge of home health compliance and documentation standards
• Excellent interpersonal skills and patient-focused care approach

Schedule and Location:

• Full-time contract position
• Monday through Friday, 8:00 AM to 5:00 PM with occasional flexibility for patient needs
• Field-based nursing visits within the Bedford, NH area
