Schedule/Hours:

Required Qualifications:

  • Active LPN license
  • A minimum of one year of recent home health experience
  • Proficiency with HomeCare HomeBase (HCHB) EMR
  • Excellent clinical assessment and documentation abilities
  • Reliable transportation for patient home visits
  • Ability to work independently and manage your caseload efficiently

Key Responsibilities:

  • Deliver skilled nursing to patients following individualized care plans
  • Monitor patient progress, update documentation, and modify care plans as needed
  • Administer medications and perform treatments within state guidelines
  • Educate patients and their families on self-care and disease prevention
  • Maintain thorough records and communications using HomeCare HomeBase (HCHB)
  • Collaborate effectively with RNs, therapists, and the healthcare team to ensure coordinated care
