Schedule/Hours:

Key Responsibilities:

  • Deliver skilled nursing care to patients at home based on individualized care plans
  • Monitor and document patient progress, updating care protocols as necessary
  • Administer medications and perform medical treatments in compliance with state regulations
  • Educate patients and their families on ongoing care management and disease prevention strategies
  • Maintain accurate, up-to-date records utilizing HomeCare HomeBase (HCHB) EMR software
  • Collaborate with registered nurses, therapists, and other healthcare professionals to ensure comprehensive care coordination

Qualifications & Experience:

  • Current, active LPN license
  • At least one year of recent home health experience required
  • Proficiency with HomeCare HomeBase (HCHB) electronic medical records system
  • Excellent clinical assessment and documentation abilities
  • Reliable vehicle for local home visits
  • Strong ability to work independently and handle multiple patient cases
