Job Description

As a Licensed Practical Nurse specializing in home health, you play a vital role in delivering compassionate, high-quality care to geriatric patients in their residences. Schedule/Hours:

Key Qualifications:

  • Active and unrestricted LPN license
  • At least one year of recent experience in home health nursing
  • Confident proficiency with HomeCare HomeBase (HCHB) EMR system
  • Strong abilities in clinical assessment, documentation, and patient communication
  • Able to work independently and manage a caseload with excellent time management
  • Reliable transportation for home visits

Essential Responsibilities:

  • Deliver skilled nursing care in accordance with individualized care plans
  • Monitor and document patients progress, adjusting plans as necessary
  • Administer medications and perform treatments following state guidelines
  • Educate patients and family members about disease management and preventative care
  • Ensure accurate recordkeeping using the HomeCare HomeBase (HCHB) platform
  • Coordinate with multidisciplinary team membersRNs, therapists, and allied professionalsto provide holistic patient care
