As a Licensed Practical Nurse specializing in home health, you play a vital role in delivering compassionate, high-quality care to geriatric patients in their residences. Schedule/Hours:
- Contract position with potential for extension
Key Qualifications:
- Active and unrestricted LPN license
- At least one year of recent experience in home health nursing
- Confident proficiency with HomeCare HomeBase (HCHB) EMR system
- Strong abilities in clinical assessment, documentation, and patient communication
- Able to work independently and manage a caseload with excellent time management
- Reliable transportation for home visits
Essential Responsibilities:
- Deliver skilled nursing care in accordance with individualized care plans
- Monitor and document patients progress, adjusting plans as necessary
- Administer medications and perform treatments following state guidelines
- Educate patients and family members about disease management and preventative care
- Ensure accurate recordkeeping using the HomeCare HomeBase (HCHB) platform
- Coordinate with multidisciplinary team membersRNs, therapists, and allied professionalsto provide holistic patient care
