Job Description

Bring your expertise and compassion to the vital field of home health as a Licensed Practical Nurse (LPN). In this rewarding full-time contract role, you'll play an essential part in supporting patients health, independence, and recovery by providing skilled nursing care in their homes.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Occasional schedule flexibility to accommodate patient needs

Key Qualifications:

  • Active, unrestricted LPN license
  • Minimum of 1 years experience in home health, hospice, or a similar setting
  • Proficient with Home Care Home Base (HCHB) charting system
  • Valid drivers license and reliable transportation with current insurance
  • Current BLS (Basic Life Support) certification
  • Excellent clinical assessment, problem-solving, and communication skills
  • Comfort working independently in a field-based role

Preferred Experience:

  • Previous work with Medicare-certified home health agencies
  • Familiarity with home health compliance and documentation standards
  • Commitment to patient-centered care and strong interpersonal skills

Day-to-Day Responsibilities:

  • Manage a caseload of 2532 points weekly (about 35 visits per day)
  • Complete skilled nursing assessments and develop individualized care plans
  • Administer medications, perform wound care, and manage IV therapies as ordered
  • Educate patients and caregivers on disease management and self-care
  • Maintain accurate electronic documentation with HCHB
  • Collaborate with an interdisciplinary team to coordinate care
  • Uphold infection control and patient safety protocols
  • Participate in case conferences, training, and quality initiatives
