Bring your expertise and compassion to the vital field of home health as a Licensed Practical Nurse (LPN). In this rewarding full-time contract role, you'll play an essential part in supporting patients health, independence, and recovery by providing skilled nursing care in their homes.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional schedule flexibility to accommodate patient needs
Key Qualifications:
- Active, unrestricted LPN license
- Minimum of 1 years experience in home health, hospice, or a similar setting
- Proficient with Home Care Home Base (HCHB) charting system
- Valid drivers license and reliable transportation with current insurance
- Current BLS (Basic Life Support) certification
- Excellent clinical assessment, problem-solving, and communication skills
- Comfort working independently in a field-based role
Preferred Experience:
- Previous work with Medicare-certified home health agencies
- Familiarity with home health compliance and documentation standards
- Commitment to patient-centered care and strong interpersonal skills
Day-to-Day Responsibilities:
- Manage a caseload of 2532 points weekly (about 35 visits per day)
- Complete skilled nursing assessments and develop individualized care plans
- Administer medications, perform wound care, and manage IV therapies as ordered
- Educate patients and caregivers on disease management and self-care
- Maintain accurate electronic documentation with HCHB
- Collaborate with an interdisciplinary team to coordinate care
- Uphold infection control and patient safety protocols
- Participate in case conferences, training, and quality initiatives
