Advance your nursing career in a dynamic home health environment where patient-centered care and clinical skills are highly valued. This full-time contract opportunity allows you to make a direct impact by providing in-home nursing care while fostering patient independence and recovery.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional flexibility based on patient needs
Key Responsibilities:
- Conduct skilled nursing visits to assess, plan, and implement individualized care plans
- Administer medications, perform wound care, and manage IV therapy and other clinical interventions as ordered
- Monitor and promptly communicate changes in patients conditions to the RN Case Manager or physician
- Educate patients and caregivers on disease processes, medication management, and self-care
- Complete accurate, timely documentation for all assessments, interventions, and communication using Home Care Home Base (HCHB)
- Collaborate with an interdisciplinary team to coordinate holistic, compliant patient care
- Maintain infection control standards and ensure safety in patients homes
- Participate in case conferences, quality improvement initiatives, and ongoing training
Qualifications:
- Active and unrestricted LPN license in the state of practice
- At least 1 year of experience in home health, hospice, or a similar community-based setting
- Proficient with Home Care Home Base (HCHB) charting system
- Current BLS certification
- Valid drivers license, reliable transportation, and up-to-date auto insurance
- Strong organizational, time management, and communication abilities
- Ability to work independently while managing a dynamic caseload
Preferred:
- Experience in Medicare-certified home health agencies
- In-depth knowledge of home health compliance and documentation standards
- Excellent interpersonal and patient-education skills
