Job Description

Deliver high-quality patient care in the home setting as a Licensed Practical Nurse, supporting recovery and independence for individuals requiring skilled nursing services. This field-based contract position is ideal for a detail-oriented professional with strong clinical and communication skills, committed to patient-centered outcomes.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Occasional flexibility based on patient needs
  • Manage 2532 points per week (generally 35 visits daily)

Key Qualifications:

  • Active, unrestricted LPN license in the state of practice
  • Minimum 1 year of home health, hospice, or related community-based care experience
  • Demonstrated proficiency in Home Care Home Base (HCHB) electronic charting
  • Current BLS certification
  • Valid drivers license, reliable transportation, and up-to-date vehicle insurance
  • Ability to work independently, prioritize tasks, and respond effectively in a dynamic home care environment
  • Strong clinical judgment and communication skills

Preferred Experience:

  • Background in Medicare-certified home health agencies
  • In-depth understanding of compliance and documentation requirements for home health
  • Excellent interpersonal skills, with a focus on patient education and advocacy

Responsibilities:

  • Conduct skilled nursing home visits, assess patient needs, and implement individualized care plans
  • Perform medication administration, wound care, IV therapy, and other clinical interventions as ordered
  • Monitor and document patient conditions, reporting changes to the supervising RN or physician
  • Provide education to patients and families on disease processes and self-management skills
  • Maintain accurate electronic documentation in HCHB
  • Collaborate with interdisciplinary team members to coordinate and optimize care
  • Adhere to infection control and patient safety guidelines
  • Attend case conferences, participate in quality initiatives, and maintain required training
