Deliver skilled nursing care directly in patients homes as a Home Health Licensed Practical Nurse (LPN), providing compassionate, patient-centered support to promote independence, recovery, and education. This full-time contract role involves managing a caseload of approximately 35 patient visits per day within an assigned territory. The position requires adherence to regulatory and organizational standards through thorough documentation and collaboration with an interdisciplinary healthcare team.

Responsibilities:

• Conduct thorough skilled nursing visits to assess, plan, and implement individualized care plans
• Administer medications, perform wound care, and manage IV therapy or other clinical interventions
• Monitor patient conditions and communicate changes promptly to RN Case Managers or physicians
• Educate patients and caregivers on disease management, medication use, and self-care practices
• Accurately document all assessments and care activities using Home Care Home Base (HCHB)
• Collaborate with interdisciplinary teams to ensure coordinated, compliant patient care
• Uphold infection control standards and maintain patient safety
• Engage in case conferences, quality improvement, and mandatory training sessions

Qualifications:

• Active, unrestricted LPN license in the state of practice
• Minimum one year of experience in home health, hospice, or similar community-based care
• Proficiency with Home Care Home Base (HCHB) electronic charting
• Valid drivers license, reliable insured vehicle, and dependable transportation
• Current Basic Life Support (BLS) certification
• Strong clinical judgment, time management, and communication skills
• Ability to work independently in a field-based environment managing dynamic caseloads

Preferred:

• Experience with Medicare-certified home health agencies
• Sound knowledge of home health compliance and documentation standards
• Excellent interpersonal and patient-focused care approach

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM with occasional flexibility for patient needs
• Caseload approximately 2532 points weekly (about 35 visits per day)
