Job Description
Provide compassionate, high-quality nursing care as a Home Health Licensed Practical Nurse (LPN) focusing on patient-centered services in home settings. This full-time contract role requires managing a caseload of patients throughout the Augusta, GA area, delivering assessments, medication administration, wound care, and education to promote patient recovery and independence.
Key Responsibilities:
- Conduct skilled nursing visits to implement individualized care plans
- Administer medications, perform wound care, and manage IV therapy as ordered
- Monitor patient conditions and report findings to healthcare team members
- Educate patients and caregivers on disease processes and self-care techniques
- Maintain accurate documentation using Home Care Home Base (HCHB)
- Collaborate with interdisciplinary teams to ensure coordinated care and compliance
- Maintain infection control and patient safety standards
- Participate in case conferences and quality improvement activities
Qualifications:
- Active, unrestricted LPN license in the state of practice
- Minimum 1 year of experience in home health, hospice, or community-based care
- Proficient in the Home Care Home Base (HCHB) charting system
- Valid drivers license, reliable vehicle, and current insurance
- Current Basic Life Support (BLS) certification
- Strong clinical judgment, time management, and communication skills
- Ability to work independently in a field-based role managing a dynamic caseload
Preferred:
- Experience with Medicare-certified home health agencies
- In-depth knowledge of home health compliance and documentation standards
- Excellent interpersonal skills with a patient-focused care approach
Schedule:
- Monday to Friday, 8:00 AM 5:00 PM with occasional flexibility to meet patient needs
