Job Description
A skilled and compassionate Licensed Practical Nurse (LPN) is sought for a field-based home health role, delivering individualized, patient-focused care in clients homes. This contract opportunity offers consistent weekday hours and the chance to make a meaningful difference as part of an interdisciplinary team.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM (occasional flexibility for patient needs)
- Full-time contract
- 2532 points per week (approximately 35 patient visits per day)
- Field-based, servicing patients within an assigned territory
Key Responsibilities:
- Conduct skilled nursing visits, assess patient needs, and implement care plans tailored to each individual
- Administer medications, perform wound care, and manage IV therapy or other clinical interventions
- Monitor and document changes in patient conditions, reporting promptly to RN Case Managers or physicians
- Educate patients and families on disease processes, medication protocols, and self-care strategies
- Accurately complete documentation using Home Care Home Base (HCHB)
- Collaborate with other healthcare professionals as part of an interdisciplinary team
- Uphold infection control standards and ensure patient safety
- Engage in required meetings, training, and quality improvement activities
Required Qualifications:
- Active, unrestricted LPN license in Washington
- At least 1 year of recent experience in home health, hospice, or community-based care settings
- Proficiency in using Home Care Home Base (HCHB)
- Valid drivers license with reliable transportation and current insurance
- Current BLS (Basic Life Support) certification
- Strong time management, communication, and clinical skills
- Ability to work independently in the field and manage a diverse caseload
Preferred Experience:
- Background in Medicare-certified home health agencies
- In-depth knowledge of home health compliance and documentation standards
- Demonstrated patient-centered care and strong interpersonal skills
