Job Description

Experienced and compassionate Licensed Practical Nurses are needed to provide skilled home health care to patients in their residences. In this dynamic, field-based contract assignment, you will deliver essential nursing services, empower patients and their families, and ensure compliant, high-quality care in collaboration with a multidisciplinary team.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM (occasional flexibility for patient needs)
  • Manage a weekly workload equivalent to 2532 points (about 35 patient visits per day)
  • Field-based travel within an assigned territory

Key Responsibilities:

  • Deliver skilled nursing assessments and individualized care for home health patients
  • Administer medications, provide wound care, and manage IV therapy as directed
  • Monitor and report patient status changes to the RN Case Manager or physician
  • Educate patients and caregivers regarding disease management, medications, and self-care
  • Accurately document all interventions and communications in Home Care Home Base (HCHB)
  • Maintain infection control and prioritize patient safety
  • Collaborate with the interdisciplinary healthcare team for care planning and compliance
  • Participate in training and quality improvement activities

Required Qualifications:

  • Unrestricted LPN license in the state of practice
  • At least 1 year of home health, hospice, or community-based care experience
  • Proficiency with Home Care Home Base (HCHB) documentation software
  • Current drivers license, reliable transportation, and automobile insurance
  • BLS (Basic Life Support) certification
  • Strong clinical judgment and excellent organizational and communication skills
  • Ability to work autonomously and efficiently in the field

Preferred:

  • Previous experience with Medicare-certified home health agencies
  • Deep knowledge of home health compliance and documentation
