Job Description

As a Home Health Licensed Practical Nurse (LPN), you will directly support patients in their homes by managing individualized care plans and fostering patient independence, education, and recovery.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
- Occasional flexibility to accommodate patient needs
Key Responsibilities:
- Perform skilled nursing visits to assess, plan, and implement patient-specific care plans
- Administer medications, wound care, and IV therapy as ordered
- Monitor patient changes, promptly communicating findings to the RN Case Manager or physician
- Provide patient and caregiver education regarding condition management and self-care strategies
- Ensure timely, accurate documentation using Home Care Home Base (HCHB)
- Coordinate with the interdisciplinary care team to optimize outcomes and compliance
- Uphold infection control and safety standards in all care settings
- Participate in case conferences, quality improvement initiatives, and ongoing training
Qualifications & Experience:
- Active, unrestricted LPN license in the state of practice
- At least 1 year of direct home health, hospice, or community-based care experience
- Demonstrated proficiency with Home Care Home Base (HCHB)
- Valid drivers license, reliable transportation, and current vehicle insurance
- BLS (Basic Life Support) certification
- Solid skills in clinical judgment, organization, and communication
- Self-motivation to work independently, managing a dynamic field-based caseload
Preferred Background:
- Prior Medicare-certified home health agency experience
- Strong understanding of home health compliance/documentation
- Professional and empathetic patient relationship skills
