Job Description
Deliver skilled nursing care as a Home Health Licensed Practical Nurse (LPN) providing patient-centered services within the comfort of patients homes. This full-time contract role is ideal for an experienced LPN passionate about home health nursing and adept at managing clinical care in a community-based setting.
Responsibilities include conducting skilled nursing visits to assess, plan, and implement personalized care plans, performing wound care, medication administration, and managing IV therapy as prescribed. You will monitor patient status changes, educate patients and caregivers on treatment and self-care, and maintain accurate documentation using Home Care Home Base (HCHB). Collaboration with interdisciplinary team members ensures coordinated, compliant care while maintaining rigorous infection control standards and patient safety.
Qualifications & Desired Experience:
- Active and unrestricted LPN license in the state of practice
- Minimum 1 year home health, hospice, or community-based care experience
- Proficiency with Home Care Home Base (HCHB) electronic charting system
- Current BLS (Basic Life Support) certification
- Valid drivers license, reliable insured vehicle, and dependable transportation
- Strong clinical judgment, communication, and time management skills
- Ability to work independently managing a dynamic caseload in the field
- Preferred: Prior Medicare-certified home health agency experience and knowledge of home health compliance and documentation standards
Schedule & Location:
- Monday Friday, 8:00 AM 5:00 PM, with occasional flexibility to meet patient needs
- Approximately 2532 points weekly, equating to 35 patient visits per day
- Field-based within assigned territory in Atlanta, GAreliable transportation required with mileage reimbursement generally provided
