A skilled Home Health Registered Nurse (RN) is needed to deliver compassionate and high-quality nursing care to patients in their homes throughout Atlanta, GA. This full-time, 13-week contract role involves managing a caseload averaging 2532 points weekly, typically 35 patient visits per day, Monday through Friday from 8:00 AM to 5:00 PM. The RN will be responsible for conducting comprehensive assessments, developing individualized care plans, and collaborating closely with interdisciplinary teams to promote patient wellness and adherence to care goals.

Key Responsibilities:

• Perform comprehensive OASIS assessments and establish patient-centered care plans.
• Deliver skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, promptly report condition changes to physicians and care teams.
• Educate patients and families regarding diseases, treatments, medication regimens, and preventive measures.
• Maintain accurate, timely documentation using Home Care Home Base (HCHB) electronic system.
• Coordinate care with therapists, social workers, and home health aides to ensure seamless service delivery.
• Participate in case conferences, staff meetings, and ongoing professional development.
• Adhere to regulatory requirements and agency standards for compliance and quality care.

Qualifications:

• Active RN license in good standing for the state of practice.
• Minimum 1 year of recent home health nursing experience; field experience preferred.
• Proficient in Home Care Home Base (HCHB) and skilled in OASIS assessments.
• Strong clinical assessment, critical thinking, communication, and organizational skills.
• Reliable transportation, valid drivers license, and auto insurance required.
• Ability to independently manage a flexible field-based schedule. Preferred:
• Bachelor of Science in Nursing (BSN).
• Experience managing 2532 points per week.
• Familiarity with Medicare and home health regulatory standards.
