Schedule/Hours:

• Flexible daytime scheduling to accommodate both patient and nurse needs

Key Qualifications:

• Current, active LPN license in good standing
• Experience providing nursing care in a home health environment highly preferred
• Proven ability to perform assessments, administer treatments, and monitor patient progress
• Exceptional communication and interpersonal skills, with a compassionate, patient-centered approach
• Basic life support (BLS) certification
• Valid drivers license and reliable transportation for home visits

Primary Responsibilities:

• Deliver direct nursing services to patients in their homes, ensuring safe, effective, and respectful care
• Conduct comprehensive patient assessments and help update individualized care plans in coordination with the healthcare team
• Administer medications, wound care, and other prescribed treatments
• Track and document patient status and interventions accurately in electronic health records
• Educate patients and family members on disease processes, treatments, and self-care techniques
• Collaborate with physicians, RNs, and other health professionals to optimize patient outcomes
• Maintain compliance with state and federal regulations, as well as company policies
