Job Description

Deliver expert nursing care as a Home Health Licensed Practical Nurse (LPN) providing patient-centered services directly in the comfort of patients homes. This role is essential in promoting patient independence, education, and recovery through personalized care and clear communication with patients, their families, and the healthcare team. The position offers a full-time contract opportunity with a structured schedule and the chance to make a tangible difference in community-based healthcare.

Key Responsibilities:

  • Conduct skilled nursing visits to assess, plan, and implement individualized care plans for home health patients.
  • Administer medications, wound care, and clinical interventions such as IV therapy per physician orders.
  • Monitor patient conditions and communicate promptly with RN Case Manager or physicians.
  • Educate patients and caregivers on disease management, medication adherence, and self-care.
  • Complete detailed documentation using Home Care Home Base (HCHB) system.
  • Collaborate with interdisciplinary teams to ensure compliance with care plans.
  • Maintain infection control and ensure patient safety.
  • Engage in quality improvement initiatives and training sessions.

Qualifications & Experience:

  • Active, unrestricted LPN license in the state of practice.
  • Minimum 1 year of home health, hospice, or community-based care experience.
  • Proficient in Home Care Home Base (HCHB) electronic charting.
  • Valid drivers license with reliable insured vehicle.
  • Current BLS (Basic Life Support) certification.
  • Strong clinical judgment, communication, and time management skills.
  • Ability to work independently managing a dynamic caseload.

Preferred:

  • Familiarity with Medicare-certified home health agencies.
  • Knowledge of home health compliance and documentation standards.
  • Patient-centered care approach and excellent interpersonal skills.

Contract Details:

  • Monday through Friday, 8:00 AM to 5:00 PM, with occasional schedule flexibility.
  • 2532 points weekly workload (approximately 35 patient visits daily).
  • Field-based work within the assigned territory.
  • Mileage reimbursement generally provided.
