Schedule/Hours:

Qualifications:
- Active LPN license in good standing
- At least one year of recent experience in a home health setting
- Proven proficiency in HomeCare HomeBase (HCHB) EMR platform
- Strong skills in clinical assessment, treatment, and documentation
- Reliable transportation required for patient home visits
- Self-motivated, able to work independently, and manage a caseload efficiently
Key Responsibilities:
- Deliver skilled nursing care tailored to each patients care plan
- Monitor patient conditions, update records, and adjust care as needed
- Administer medications and perform treatments in compliance with state guidelines
- Educate patients and families about disease management and prevention
- Maintain detailed, timely records using the HomeCare HomeBase (HCHB) EMR system
- Collaborate with RNs, therapists, and other professionals to ensure holistic patient care
