Bring your expertise in home health nursing to a role where you can make a genuine difference in patients lives. This contract opportunity seeks an experienced and compassionate Licensed Practical Nurse (LPN) skilled in delivering patient-centered care in a home-based setting.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional flexibility to accommodate patient needs
Key Responsibilities:
- Conduct skilled nursing visits to assess, plan, and implement individualized home health care plans
- Administer medications, perform wound care, and manage IV therapy or other clinical treatments as ordered
- Monitor for and promptly report any changes in patients conditions to the RN Case Manager or physician
- Educate patients and families on disease management, medication administration, and self-care techniques
- Complete all documentation and care records accurately within Home Care Home Base (HCHB)
- Collaborate closely with the interdisciplinary healthcare team to coordinate care and ensure compliance with established care plans
- Maintain excellent infection control standards and prioritize patient safety during all interactions
- Participate in case conferences, quality improvement activities, and mandatory training sessions
Required Qualifications:
- Active and unrestricted LPN license in the state of practice
- At least 1 year of experience in home health, hospice, or similar field-based nursing role
- Proficiency with Home Care Home Base (HCHB) software
- Current drivers license, reliable transportation, and valid insurance
- Current BLS certification
- Ability to work independently and manage a caseload of 2532 points weekly (approximately 35 patient visits daily)
- Strong clinical, time management, and communication skills
Preferred Experience:
- Background in Medicare-certified home health agencies
- Deep understanding of home health compliance and documentation requirements
- Demonstrated patient-focused approach and strong interpersonal abilities
