As a Home Health Licensed Practical Nurse (LPN), you will deliver high-quality, patient-centered care to individuals in their homes. Your nursing expertise will foster patient independence, education, and recovery, while effective communication with patients, families, and healthcare professionals will drive positive outcomes. This contract position is ideal for LPNs dedicated to clinical excellence, seeking to advance their skills in a supportive, field-based environment.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional flexibility for patient needs
Key Responsibilities:
- Conduct skilled nursing visits and assessments, creating and implementing individualized care plans.
- Administer medications, perform wound care, and manage clinical interventions such as IV therapy, as ordered.
- Monitor and promptly report changes in patient conditions to the RN Case Manager or physician.
- Deliver education to patients and caregivers on disease management and self-care.
- Complete accurate documentation of patient visits, care, and communication within Home Care Home Base (HCHB).
- Coordinate with the interdisciplinary team for seamless, compliant patient care.
- Uphold infection control standards and prioritize patient safety.
- Participate in case conferences, quality improvement initiatives, and training sessions.
Required Qualifications:
- Active, unrestricted LPN license in the state of practice.
- Minimum of 1 year of home health, hospice, or related community-based care experience.
- Proficiency with Home Care Home Base (HCHB).
- Valid drivers license, reliable vehicle, and current insurance.
- Current BLS (Basic Life Support) certification.
- Strong clinical judgment, communication, and time management skills.
- Ability to work independently, managing a dynamic caseload.
Preferred Experience:
- Work with Medicare-certified home health agencies.
- In-depth knowledge of home health compliance and documentation standards.
- Exceptional patient-focused and interpersonal skills.
