Job Description

A rewarding opportunity is available for an experienced Home Health Licensed Practical Nurse (LPN) to deliver patient-centered care in a field-based setting. This contract role offers the chance to promote independence and recovery for patients in their homes, while working collaboratively within an interdisciplinary healthcare environment.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Occasional flexibility based on patient needs

Responsibilities:

  • Conduct skilled nursing visits, develop and implement care plans
  • Deliver clinical interventions such as medication administration, wound care, and IV therapy
  • Monitor and document changes in patient conditions, communicating promptly with RN Case Manager or physician as appropriate
  • Educate patients and caregivers on disease management, medications, and self-care
  • Document all care and communications using Home Care Home Base (HCHB)
  • Collaborate with other healthcare professionals to ensure a coordinated plan of care
  • Maintain infection control and safety protocols at all times
  • Participate in case conferences, ongoing training, and quality improvement initiatives

Qualifications:

  • Active, unrestricted LPN license in the state of practice
  • Minimum of 1 year of relevant experience in home health, hospice, or similar settings
  • Proficiency in Home Care Home Base (HCHB)
  • Valid drivers license, reliable insured vehicle
  • Current BLS certification
  • Strong clinical assessment and organizational skills, ability to work independently

Preferred:

  • Experience with Medicare-certified home health agencies
  • Detailed understanding of home health compliance protocols
  • Exceptional interpersonal and patient education skills
