Job Description
A rewarding opportunity is available for an experienced Home Health Licensed Practical Nurse (LPN) to deliver patient-centered care in a field-based setting. This contract role offers the chance to promote independence and recovery for patients in their homes, while working collaboratively within an interdisciplinary healthcare environment.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional flexibility based on patient needs
Responsibilities:
- Conduct skilled nursing visits, develop and implement care plans
- Deliver clinical interventions such as medication administration, wound care, and IV therapy
- Monitor and document changes in patient conditions, communicating promptly with RN Case Manager or physician as appropriate
- Educate patients and caregivers on disease management, medications, and self-care
- Document all care and communications using Home Care Home Base (HCHB)
- Collaborate with other healthcare professionals to ensure a coordinated plan of care
- Maintain infection control and safety protocols at all times
- Participate in case conferences, ongoing training, and quality improvement initiatives
Qualifications:
- Active, unrestricted LPN license in the state of practice
- Minimum of 1 year of relevant experience in home health, hospice, or similar settings
- Proficiency in Home Care Home Base (HCHB)
- Valid drivers license, reliable insured vehicle
- Current BLS certification
- Strong clinical assessment and organizational skills, ability to work independently
Preferred:
- Experience with Medicare-certified home health agencies
- Detailed understanding of home health compliance protocols
- Exceptional interpersonal and patient education skills
