Job Description
Deliver compassionate, high-quality care in the community as a Home Health Licensed Practical Nurse (LPN), supporting patients in their journey toward greater independence and wellness. In this field-based contract position, youll play a vital role in providing skilled nursing services and education to individuals in their homes, ensuring continuity of care and empowering patients and families.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional flexibility for patient needs
Key Responsibilities:
- Conduct home visits, assessing and implementing individualized patient care plans
- Administer medications, wound care, IV therapy, and other clinical interventions as ordered
- Monitor and document changes in patient conditions, reporting to the RN Case Manager or physician
- Educate patients and caregivers on disease management, medications, and self-care techniques
- Maintain detailed and compliant documentation using Home Care Home Base (HCHB)
- Coordinate with the interdisciplinary team and participate in case conferences and quality improvement activities
- Uphold infection control practices and prioritize safety
Qualifications:
- Active, unrestricted LPN license in the state of practice
- At least 1 year of home health, hospice, or similar experience
- Proficiency with Home Care Home Base (HCHB)
- Valid drivers license, reliable vehicle, and current insurance
- BLS certification
- Strong clinical judgment, communication, and organizational skills
- Self-motivated and able to manage a dynamic caseload independently
Preferred:
- Medicare-certified home health experience
- Excellent interpersonal skills and knowledge of compliance standards
