Key Responsibilities:

Collaborate closely with healthcare professionals to develop comprehensive home care treatment plans.
Diligently record and monitor patient conditions and progress.
Administer medications and provide essential wound care as necessary.
Ensure accurate documentation using relevant systems, including HomeCare HomeBase (HCHB) and OASIS Platforms.

Requirements:

Active RN licensure in Pennsylvania.
Minimum of one year of recent experience in home health.
Previous experience managing a home-to-home workload is advantageous.
Familiarity with HomeCare HomeBase (HCHB) and OASIS Platforms preferred.
Current BLS Certification from the American Heart Association.

Details:

Engage in fulfilling home-to-home responsibilities within the Canonsburg, PA area.
Full-time role, 40 hours per week, with shifts scheduled from 8:00 AM to 5:00 PM.
Mileage reimbursement provided for travel within Canonsburg.
