Join Our Team as a Home Health RN in Franklin, Louisiana!
Are you an experienced Registered Nurse (RN) with a heart for providing compassionate care? We're actively seeking a dedicated RN to join our team for a 13-week contract in home health. If you have recent experience in home health, proficiency in OASIS and Homecare Homebase (HCHB), solid clinical skills, and a genuine commitment to patient well-being, we want to hear from you!

Your Responsibilities:

Collaborate with interdisciplinary teams to develop personalized home care treatment plans.
Document patient assessments and progress using OASIS and HCHB systems.
Administer medication and provide wound care as needed.
Ensure accurate documentation and reporting utilizing OASIS and HCHB expertise.

Requirements:

Active RN licensure in Louisiana.
Minimum one year of recent home health experience.
Proficiency in OASIS and Homecare Homebase (HCHB) systems.
Current BLS Certification from the American Heart Association.
Previous experience managing a home-to-home workload is advantageous.

Additional Details:

Mileage reimbursement provided for travel within Franklin.
