Job Description

Sunbelt is seeking a Registered Nurse for a Home Health contract position with a local client near Terre Haute, IN. Home Health RN’s with at least one year of recent home health experience are encouraged to apply for this opportunity. Employment Type: Full-time, 13 week contract with potential to extend
Works Hours: 8:00AM-5:00PM, 40 hours per week, some on call and weekends required

Responsibilities:

Utilize expert knowledge of OASIS-C Assessment Documentation Start of Cares/Admissions, Recertification’s, Resumptions of Care, Transfers, and Discharges
Complete accurate and detailed OASIS-C Documentation within a 24-48 hour turnaround time
Case manage at least 5-7 patients per day and 25-30 patients per week
Scheduled visits will include, but are not limited to:
- Providing wound care
- IV administration and teaching
- Cardiac program follow up and intervention
Travel in the Terre Haute, IN area

Experience:

At least one year of Home Health experience within the last year required
In depth knowledge and in-field experience of OASIS-C required
Start of Care/Admissions experience required
Experience case managing at least 5-7 patients per day and 25-30 per week

Education:

Associates Degree of Nursing or Bachelors Degree of Nursing
Indiana Registered Nurse License
American Heart Association Basic Life Support
