Seeking Compassionate Home Health Registered Nurse for home health contract in O'Fallen

Responsibilities:

Conduct comprehensive patient assessments, develop individualized care plans, and provide skilled nursing interventions to promote optimal health outcomes.

Educate patients and their families on disease management, medication administration, and self-care techniques to empower them in their health journey.

Requirements:

Valid Registered Nurse (RN) license
Minimum of 1 year of home health experience with OASIS
Proficiency in conducting comprehensive assessments, developing care plans, and administering skilled nursing interventions.
Strong communication, interpersonal, and organizational skills.
Comfortable utilizing technology for documentation, communication, and remote patient monitoring.
