Responsibilities:

Collaborate closely with healthcare professionals to develop comprehensive home care treatment plans.
Diligently monitor and record patient conditions and progress.
Administer medications and provide essential wound care.
Ensure accurate documentation using Oasis and HomeCare HomeBase.

Requirements:

Active RN licensure in Tennessee.
Minimum of one year of recent home health experience.
Proficiency in Oasis and HomeCare HomeBase.
Previous experience with home-to-home workload is advantageous.
Current BLS Certification from the American Heart Association.

Details:

Full-time positions, 40 hours per week.
Shifts scheduled from 8:00 AM to 5:00 PM.
Mileage reimbursement available for travel within Cordova.
