Job Description

Are you a dedicated and experienced Licensed Practical Nurse (LPN) seeking a fulfilling opportunity in home health care in Augusta, Georgia? We're actively seeking a compassionate LPN for an engaging 13-week contract. If you possess at least one year of recent experience in home health, along with proficiency in HomeCare HomeBase (HCHB) experience, we'd love to hear from you!

Responsibilities:

Collaborate closely with healthcare professionals to develop comprehensive home care treatment plans.
Diligently record and monitor patient conditions and progress.
Administer medications and provide essential wound care as necessary.
Ensure accurate documentation using relevant systems, including HomeCare HomeBase (HCHB).

Requirements:

Active LPN licensure in Georgia.
Minimum of one year of recent experience in home health.
Proficiency in HomeCare HomeBase (HCHB).
Previous experience managing a home-to-home workload is advantageous.
Current BLS Certification from the American Heart Association.

Details:

Engage in fulfilling home-to-home responsibilities within the Augusta, GA area.
Full-time role, 40 hours per week, with shifts scheduled from 8:00 AM to 5:00 PM.
Mileage reimbursement provided for travel within Augusta.
