Job Description

There is an open Full Time Geriatric Nurse Practitioner (NP) - Long Term Care job in Greenwich, CT. The Full Time Geriatric Nurse Practitioner (NP) will be responsible for acute and chronic disease management for assisted living and senior housing residents. The Geriatric Nurse Practitioner (NP) will conduct a one time annual insurance assessment and partner with the PCP and long term care staff to provide primary and preventative care while proactively identifying triggers for acute illness and chronic conditions. Depending on acuity the NP will perform 5-7 assessments per day with a goal to improve the overall quality of care and decrease the need for avoidable hospitalizations. The assessment will be conducted on Medicare Advantage plan members residing in long term care and skilled nursing facilities in Greenwich, CT and the surrounding Fairfield county area. The NP is typically scheduled in one facility per shift.

Responsibilities:

Conduct annual comprehensive assessments for a Medicare Advantage health plan
Communicate and collaborate with the interdisciplinary team
Provide education on medical conditions and medications
Conduct advanced illness and advanced care planning conversations

Work Hours:

Monday and Friday
8:00 AM - 5:00 PM

Preferred Experience:

1 year of current NP experience is preferred

Preferred Qualifications:

Nurse Practitioner (NP) License in CT
Active ANCC or AANP certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care
DEA
Current CPR certification (BLS)
COVID vaccination required

Education Requirements:

MSN degree from an accredited school of nursing
