Job Description

Sunbelt Staffing, a nationwide healthcare staffing agency, is seeking a compassionate and experienced Home Health Registered Nurse (RN) for a 13-week contract position in Ville Platte, LA . The successful candidate will join our client, a leading nationwide home health agency, and provide quality patient care in a home-based setting.

Contract Details:

Location: Ville Platte, LA
Company: Sunbelt Staffing
Contract Length: 13 Weeks (with possible extension upon successful start and completion)
Hours: Full-time, 40 hours per week, Monday-Friday, 8am-5pm, with occasional weekends and on-call duty (typically one weekend per month on-call)
Compensation: Negotiable, based on experience and territory budget
Mileage Reimbursement: 0.48c per mile from patient home to home
Blended Pay Packages: Available for travelers
Benefits: Medical, dental, vision, 401k match after 90 days

Job Responsibilities

Perform comprehensive patient assessments using OASIS, ensuring accurate documentation and compliance with agency and regulatory standards.
Develop and implement individualized care plans, coordinating with physicians and other healthcare professionals.
Administer medications, monitor vital signs, and provide skilled nursing care, including wound care, catheter care, and IV therapy.
Educate patients and their families on care plans, health management, and preventive measures.
Maintain accurate patient records in Homecare Homebase (HCHB).
Travel to patient homes within the Ville Platte, LA area, utilizing reliable transportation.
Participate in case conferences and team meetings to ensure coordinated and effective care.
Comply with Joint Commission standards and other regulatory requirements.

Requirements

Active Registered Nurse (RN) license in the state of Louisiana.
Minimum of 1 year of home health experience.
Proficiency in OASIS and Homecare Homebase (HCHB).
Current American Heart Association or American Red Cross Basic Life Support (BLS) certification.
Reliable transportation with valid driver's license.
Ability to meet Joint Commission onboarding standards upon offer.
