About the Role: We are looking for a dedicated and compassionate Registered Nurse (RN) to join our team in New Iberia, LA, for a travel position in home health care. This role offers the chance to provide high-quality care to patients in the comfort of their own homes, while experiencing the vibrant culture of Louisiana.

Key Responsibilities: In this role, you will:

Perform thorough patient assessments and develop personalized care plans.
Administer skilled nursing care, including wound care, medication management, and patient education.
Collaborate closely with patients, families, and caregivers to ensure comprehensive care delivery.
Communicate effectively with interdisciplinary team members to coordinate patient care.
Maintain accurate documentation of patient encounters and care provided.

Requirements: To be successful in this role, you must have:

Active RN license in the state of Louisiana.
Minimum of 6 months experience in home health nursing.
Proficiency in using HomeCare/HomeBase EMR system.
Current BLS certification.
