Responsibilities:

Conduct home health visits, ensuring the delivery of high-quality care.
Adhere to productivity targets, aiming for 25 to 30 units per week.
Utilize HomeCare/HomeBase EMR system for efficient documentation.
Collaborate with interdisciplinary teams to develop and implement patient care plans.
Provide compassionate and personalized care to meet the unique needs of each patient.
Communicate effectively with patients, families, and healthcare professionals.

Requirements:

Active LPN license in Louisiana.
Minimum of 6 months experience in home health.
Proficiency in HomeCare/HomeBase EMR system.
Strong interpersonal and communication skills.
Compassionate and dedicated to providing patient-centered care.
Reliable transportation and willingness to travel between patient homes.
Commitment to maintaining high standards of professionalism and integrity.
