Job Description

Position: Licensed Practical Nurse (LPN) - Home Health

Description: Seeking a dynamic LPN for a 13-week contract position in Nashville, TN. If you're passionate about providing exceptional care in a home health setting, this role is for you. Join our team and make a positive impact on patients' lives while enjoying the vibrant energy of Music City. This position offers Monday through Friday, 8 am to 5 pm shifts, with possible PRN on weekends.

Responsibilities:

Conduct visits to both assisted living facilities and patient homes.
Meet weekly productivity targets of 25 to 35 units.
Participate in on-call duties every 4th week.
Utilize Homecare/Homebase EMR system, OASIS, and Outlook for documentation and communication.

Requirements:

Active LPN license in Tennessee.
Current BLS certification (American Heart Association or American Red Cross).
Previous home health experience required.
Proficiency in Homecare/Homebase EMR system, OASIS, and Outlook.
Availability to work Monday through Friday with possible PRN on weekends.
