Responsibilities:

Collaborate closely with a multidisciplinary team to develop comprehensive home care treatment plans.
Diligently monitor and document patient conditions and progress.
Administer medications and provide essential wound care.
Ensure meticulous documentation using HomeCare HomeBase.

Requirements:

Active LPN licensure in Florida.
Minimum of one year of recent home health experience.
Proficiency in HomeCare HomeBase.
Previous experience with a home-to-home workload is advantageous.
Current BLS Certification from the American Heart Association.

Details:

Full-time positions, 40 hours per week.
Shifts scheduled from 8:00 AM to 5:00 PM.
Mileage reimbursement available for travel within Daytona Beach.
