Job Description

Sunbelt Staffing is seeking a compassionate and experienced Hospice Home Health Registered Nurse (RN) to join our team on a 13-week contract in Akron, OH with our partnered Home Health agency. The ideal candidate will have a passion for providing high-quality patient care in a home-based setting. This position requires proficiency in OASIS documentation and experience with Homecare Homebase software.

Contract Details:

Client: Nationwide Home Health agency
Location: Akron, OH
Type: Contract (13 weeks)
Hours: Full-time, 40 hours/week, Monday-Friday (8:00am-5:00pm)
Additional Hours: Occasional weekends and on-call (typically one weekend per month)
Benefits: Medical, Dental, Vision, 401(k) after 90 days, Mileage Reimbursement, Weekly Pay
Compensation: Negotiable hourly rate based on territory budget and experience
Travelers: Blended pay packages with stipends available

Responsibilities:

Conduct in-home patient assessments and develop individualized care plans in coordination with the hospice team.
Administer medications, treatments, and therapies as prescribed.
Monitor patient progress and adjust care plans accordingly.
Communicate effectively with patients, families, and other healthcare team members.
Complete all necessary documentation using OASIS and Homecare Homebase software.
Provide patient and family education to promote understanding of hospice care.
Participate in regular team meetings to coordinate care and discuss patient needs.
Travel within the Virginia Beach area to visit patients' homes (mileage reimbursement provided).

Requirements:

Active Registered Nurse (RN) license in Ohio.
Active Basic Life Support (BLS) certification.
Minimum 1 year of home health experience and 1 year of hospice experience.
Proficiency in OASIS and Homecare Homebase software.
Reliable transportation for home visits.
Strong organizational and communication skills.
Ability to work independently and as part of a multidisciplinary team.
