As a Contract Home Health Registered Nurse, you will play a crucial role in delivering high-quality nursing care to our patients. You will have the opportunity to make a positive impact by assessing patients, developing care plans, and providing the necessary medical interventions to promote their well-being and independence.

Experience with OASIS is required for this assignment.

Responsibilities:

Conduct comprehensive patient assessments to evaluate their health status, including physical, emotional, and social factors.
Develop and implement individualized care plans based on assessment findings, physician orders, and patient goals.
Administer skilled nursing care, including wound care, medication administration, IV therapy, and other nursing procedures.
Educate patients and their families on disease management, medication regimens, and self-care techniques.
Monitor and document patient progress, making necessary adjustments to the care plan as needed.
Collaborate with interdisciplinary healthcare teams, including physicians, therapists, and social workers, to ensure holistic patient care.
Maintain accurate and timely electronic medical records in compliance with regulatory guidelines.

Requirements:

Current and unrestricted Registered Nurse (RN) license
Minimum of 6 months of geriatric home health experience
Strong clinical assessment and critical thinking skills.
Excellent communication and interpersonal skills to effectively interact with patients and their families.
Compassion and a patient-centered approach to care.
Valid driver's license and access to reliable transportation.
