Are you an experienced and compassionate Home Health RN seeking a rewarding opportunity? Look no further! We are currently seeking a dedicated individual to join our team in Glen Carbon, IL.

Position: Home Health RN

Location: Glen Carbon, IL

Responsibilities:

Conduct Point Of Care visits for wound care, IV therapy, post-surgical assessments, and more.
Provide patient education on medications, wound care, trach care, and GTubes feedings.
Perform ADLs assessment and OASIS documentation.
Ortho experience required.
Handle admission assessments, discharges, and after-hours call duties.
Proficient in wound vac, glucometer, pro-time machine PT/INR, IV pumps, and central lines/ports.

Qualifications:

1-2 years MS or acute care experience preferred.
Minimum 1 year home care experience preferred.
Ortho experience is a must.
Customer service-oriented.
Able to represent BJC Alton Home Care.
