Job Description

Description: Seeking a dedicated Home Health Registered Nurse for a 13-week contract position in Chestertown, MD. Join our team and make a difference in the lives of patients receiving home health care. This role requires experience with the HomeCare/HomeBase EMR system and a background in home health nursing. The position offers full-time hours, Monday through Friday, from 8 am to 5 pm.

Responsibilities:

Perform comprehensive assessments and develop individualized care plans for patients.
Administer medications, treatments, and wound care as prescribed.
Collaborate with interdisciplinary teams to ensure coordinated patient care.
Provide patient and family education on disease management and self-care techniques.
Document all patient interactions accurately and maintain up-to-date records.

Requirements:

Active Registered Nurse (RN) license in the state of Maryland.
Current Basic Life Support (BLS) certification from the American Heart Association or the American Red Cross.
Minimum of one year of experience in home health nursing.
Proficiency in using the HomeCare/HomeBase EMR system.
Strong communication, critical thinking, and time management skills.
