Job Description

Are you a dedicated Registered Nurse (RN) looking for a fulfilling opportunity in home health care? Join our team in Chapel Hill, NC, where we're seeking RNs with at least 2 years of experience to provide compassionate care to our patients. This full-time position offers a 13-week schedule, Monday through Friday from 8am to 5pm, with the expectation of completing 30 productivity units each week. Additionally, RNs will be on call one to two weekends a month. Reliable transportation for home visits is required.

Responsibilities:

Conduct comprehensive assessments and provide skilled nursing care to patients in their homes.
Develop and implement individualized care plans in collaboration with patients, families, and healthcare team members.
Administer medications, treatments, and procedures as prescribed.
Educate patients and families on disease management and self-care techniques.
Maintain accurate documentation of patient care activities and outcomes.

Requirements:

Active RN license in the state of North Carolina.
Minimum of 2 years of nursing experience.
Current BLS certification from the American Heart Association or the American Red Cross.
Proficiency in home health care practices and procedures.
Excellent communication and interpersonal skills.
Ability to work independently and as part of a multidisciplinary team.
Availability to be on call for weekend rotations.
