Sunbelt is seeking a License Practical Nurse (LPN) with two years of experience for a Long-Term Care and Psychiatric for a facility in Glencliff, NH. JOB SUMMARY:
Provides direct nursing care and monitors the health, safety and well being of the residents in our home-like psychiatric facility.

ESSENTIAL FUNCTIONS:
1. Visit residents when requested and as needed, conduct ongoing evaluation of current residents
2. Respond to resident emergencies; provide first aid assistance, arrange appropriate medical attention and follow up
3. Transcribe new orders on MOR, follow up on all physician orders for delivery and administration; check new MORs for accuracy
4. Conduct periodic medication audits per the Director; order medications and fax orders to pharmacy for profile updates
5. Communicate with medical professionals and resident families when there is a change in resident status or incident
6. Communicate information pertaining to changes in resident’s functional status, personal medications, appointments to the Director
7. Destroy discontinued or out of date medications, medications left by Power of Attorney after resident death or move
8. Assist with in-services as needed for Resident Aides
9. Administer staff TB tests as needed
10. Order medical supplies, sharps containers through front desk
11. Assess current residents returning from rehab or hospital with nurse/case manager
12. Administer injections to residents
13. Prepare paperwork when resident has a doctor appointment; take part in the care plan process, coordinate blood pressure reading and weight screening
14. Chart initial nurse’s note entry upon move in
15. Oversee and manage the continuity and consistency of medication training
16. Perform other duties as assigned

Minimum Requirements

  • Completion of an accredited LPN program accepted by the New Hampshire Board of Nursing for licensure
  • Current LPN license recognized by the State of NH Board of Nursing or Compact License
  • Current Basic Life Support, American Heart Association
  • One year in travel LPN experience
  • One year Psychiatric experience

Shift: Evening NOC, 8 Hour shifts, 40 Hours a week

Employment Type: Travel Contract

Housing: There is housing on campus within walking distance of the facility.
